The average hospital is not designed to handle the demands of patients requiring long-term acute care. That's why our post-acute care team at NHC Desloge is so special. Our many services are focused on improving the lives and medical conditions of patients who either need more acute care for their recovery or who are not yet ready to go home.
Post-acute care is recommended for those who require complex medical management and therapeutic care 24 hours a day, 7 days a week. We meet the special needs of patients with physical, occupational, speech, and medical nutritional therapies, combined with 24-hour nursing care. Whether provided for several weeks or several months, our physician directed services are designed to complement a hospital stay or, in many cases, be a substitute for continued hospitalization.
Post-acute care is the comprehensive, inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is a goal-oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person's underlying long-term conditions and overall situation.
At NHC Desloge, an interdisciplinary approach is used to formulate a goal-oriented plan of care for each patient. Our care team of doctors, therapists, nurses and other health professionals are dedicated to deliver a quality experience and good results to all concerned. Our focus is work with the patient and their family to assure the most efficient care to achieve their highest level of independence as quickly as possible.
